What is the difference between IBS and IBD?

Two similar-sounding names, two very different conditions. Knowing which one is being discussed changes everything that follows.

IBS — Irritable Bowel Syndrome — is functional: the bowel is structurally normal but behaves unpredictably. IBD — Inflammatory Bowel Disease, chiefly Crohn's disease and ulcerative colitis — involves genuine inflammation and damage to the bowel wall.

How they differ in practice

IBD can cause blood in the stool, fever, anaemia, unexplained weight loss and inflammation visible on investigation. IBS does not. IBD is diagnosed through testing and managed medically, often lifelong; IBS is managed largely through diet, routine and stress.

Why the distinction matters

Treating IBD as though it were IBS delays necessary medical care. Any of the warning signs above should be investigated by a doctor before assuming a functional cause.

They can overlap

Some people with well-controlled IBD still have IBS-type symptoms. Even then, the medical treatment continues — the lifestyle work sits alongside it.
